Tips on Making Sure Your Social Security Disability Application is Complete

The best way to make sure your Social Security disability application is approved is to make sure it is filled out properly and that all of the necessary documentation is included. If you are not sure what you need to complete your application, take a look below.

Filling out the application

Your first step in applying for disability benefits is to fill out the application form. Although you may fill out a paper form, it is better to apply online, using the simple application form. You can access the form by going to the Social Security Administration’s website, and clicking on the “Start a New Application” button. Then, simply fill out the form with the appropriate information Be prepared to provide the information listed below.

  • Personal information
  • Name
  • Social Security Number
  • Birth date
  • Place of Birth
  • Marriage dates
  • Divorce dates (if applicable)
  • Your bank’s routing number and the account number where you would like your payments deposited.

Also, you must provide the same information for your current spouse and any former spouses, as well as the birth dates and places of your children.

  • Medical condition
  • Name and contact information of at least one person who can attest to your medical condition
  • Detailed information about your medical condition
  • Names and contact information for all doctors, hospitals, and clinics who have treated you.
  • Any patient ID numbers you have had
  • Dates of all treatments you have received
  • Names of all medications you take and the names of the prescribing doctors
  • Names and dates of all medical tests and the names of the ordering doctors
  • Work information
  • The amount of money you earned this year and last year
  • Name and address of your employer this year and last year
  • A copy of your Social Security statement
  • Beginning and ending dates of any military service prior to 1968
  • A list of the last five jobs you have had over the past 15 years prior to your inability to work and the dates of employment of each job
  • Information regarding any filing or intent to file Worker’s Compensation or similar benefits


Along with your application, you will be asked to provide specific documentation as well.

  • Birth certificate
  • Proof of citizenship or legal alien status
  • US military discharge (if you served prior to 1968)
  • W2 forms or self-employment tax forms
  • Medical records
  • Test results
  • Award letters, pay stubs, or other proof of receipt of Worker’s Compensation or other similar benefits
