Filling out the application

Your first step in applying for disability benefits is to fill out the application form. Although you may fill out a paper form, it is better to apply online, using the simple application form. You can access the form by going to the Social Security Administration’s website, and clicking on the “Start a New Application” button. Then, simply fill out the form with the appropriate information Be prepared to provide the information listed below.

  • Personal information
  • Name
  • Social Security Number
  • Birth date
  • Place of Birth
  • Marriage dates
  • Divorce dates (if applicable)
  • Your bank’s routing number and the account number where you would like your payments deposited.

Also, you must provide the same information for your current spouse and any former spouses, as well as the birth dates and places of your children.

  • Information regarding any filing or intent to file Worker’s Compensation or similar benefits

Along with your application, you will be asked to provide specific documentation as well.

  • Proof of citizenship or legal alien status
  • US military discharge (if you served prior to 1968)
  • W2 forms or self-employment tax forms
  • Medical records
  • Test results
  • Award letters, pay stubs, or other proof of receipt of Worker’s Compensation or other similar benefits
